Extradural anterior clinoidectomy in surgical management of clinoidal meningiomas

Abstract Background Despite the recent advances in skull base surgery, microsurgical techniques, and neuroimaging, yet surgical resection of clinoidal meningiomas is still a major challenge. In this study, we present our institution experience treatment anterior highlighting role extradural clinoidectomy improving visual outcome extent tumor resection. This prospective observational study conducted on 33 consecutive patients with meningiomas. The approach utilized consisted clinoidectomy, optic canal deroofing falciform ligament opening all patients. primary assessment was improvement secondary outcomes were resection, recurrence, postoperative complications. Results included 5 males 28 females mean age 49.48 ± 11.41 years. Preoperative deficit 30 (90.9%) Optic involvement 24 (72.7%) patients, ICA encasement 16 (48.5%), cavernous sinus invasion 8 (24.2%). Vision improved 21 (70%), while 6 (20%) had stationary course 1 patient (3%) suffered new deterioration. Gross total achieved (72.7%). main factors precluding removal encasement. Mortality rate 6.1%; follow-up period 27 13 months. Conclusions series, use provided favorable meningioma
